Preparing Students For The Upcoming School Year
The Kindness Campaign has partnered with Patrick M. Magoon Institute for Healthy Communities at Ann & Robert H. Lurie Children’s Hospital of Chicago to have a mobile health unit (MHU) to serve uninsured and underinsured students on Thursday, July 29th 9am – 3pm at The Kindness Campaign’s Impact Center 703 E 75th St Chicago, IL 60619.

· Sports Physicals
· Immunizations
· Well-child Check-ups
· Health Education
YOU MUST BE CONFIRMED – NO WALK-INS.
Requirements
· Pre-K – 12th Grade Student
· Uninsured Or Underinsured
· Complete And Submit Packet By Wednesday, July 14th
